Overview Heart Failure Nurse Navigator RN - Westmont/Naperville - Cardiology ***$5,000 Sign On Bonus Included!*** Position Highlights :
- Full-Time, 40 hours/week
- Hours: Monday-Friday; 8:00-4:30pm
- Location: Westmont and Naperville
- Travel: Travel between Westmont and Naperville; Travel to other clinics for coverage as needed

Responsibilities - Escort patients to and from exam rooms and readies them for physicians; takes vital signs, records medical history/symptoms, ensures medical chart/lab test results are available and complete.
- Perform telephone triage, patient education, handling of physician phone calls and messages, pharmacy calls; may assist by calling to set up appointments or arrange hospital tests or admissions.
- Administer immunizations and minor treatments as prescribed; records and documents all such activities in the chart.
- Assist physician with procedures as necessary. Lead apron required during procedures.
- Assist with procedures including patients with conscious sedation.
- Respond to patient questions and concerns as liaison between physician and patient.
- Report all test results to the patient as directed by the physician.
- Utilize current nursing practices to provide patient care while focusing on the needs of the patient and exemplifying service excellence, care, concern, respect and cooperation.
- Additional duties as assigned.
Qualifications - Current RN licensure in nursing.
- Knowledge of nursing skills in accordance with nursing standards in the Nurse Practice Act.
- Knowledge of OSHA and HIPAA
- Prefer a minimum of 3 years of nursing experience in a clinical or hospital setting but may consider new grad under specific circumstances.
- Experience as a heart failure nurse or cardiology nurse is preferred.
The compensation for this role includes a base pay range of $38.00-50.00 with the actual pay determined by factors such as skills, experience, education, certifications, geographic location, and internal equity. Additional compensation may be available through shift differentials, bonuses, and other incentives. Base pay is only a portion of the total rewards package.
