Job Description
Full-Time Care Managers working from home office - Must be a RN or BSN. Pay is based on experience and productivity.

Job Hours are from 8:30 to 5:00pm MST.
Main Role is to complete Patient-Centric Comprehensive Care Plans based on all chronic conditions for Medicare and Medicaid Patients with multiple chronic conditions. Bilingual Speakers of Spanish, Polish, or Russian is a plus but not required.
The Plan of Care is based on a physical, mental, cognitive, psychosocial, functional and environmental (re)assessment, and an inventory of resources as well as other items to be addressed as part of the completing billing for this Medicare service.
Additionally, if needed in a month Care Manager may provider follow up Care Management calls to Medicare patients with multiple chronic conditions if needed.
This job is telephonic and employees work at their designated home office.
The Care Manager must have excellent phone skills, be comfortable calling patients to discuss care, if needed make appointments with patients to provide such care for the patient and provide education without direct supervision. Care Manager must have superior time management and communication skills, show initiative and be self- motivated. Multi-tasking is required as the Care Manager will need to be able to navigate the platform of their EHR, the internet, Microsoft Office (Excel, Word and PowerPoint) and email as well as document their time spent with each patient. A positive attitude and willingness to receive instruction and guidance from supervisor is a must.
Job duties may be expanded as needs arise. Care Manager further agrees to render and provide said work, services, labor and/or materials in accordance with the specifications in a workmanlike manner. All services are to be consistent with applicable Medicare regulations for billing CCM services.
